Get your name in my game!

As an ode to one of my favorite communities for game development and the one that got me really started off, I'd like to give everyone the option to get their name on an item in my game. I'm currently developing an ORPG with a friend, and I'm working on items right now. If you'd like to get your name on an item, for example "Bob's Sword of Ruin", just fill out this short form and post it below! Thanks.

Name (no first names such as Nick, Matt, etc.):

Class (Warrior, Mage, Rogue, Cleric):

Player Level to equip (5-40 for magic, 15-40 for rare, 30-40 for epic):

Rarity (Magic, Rare, Epic):

Type (Weapon, Armor, Accessory):

Currently the item slots are Main Hand - Off Hand - Helm - Chest - Legs - Gloves - Boots - Ring - Amulet.

If you want to make a **Warrior** weapon it can be a sword, axe, mace, polearm/spear, or shield.

If you want to make a **Mage** or **Cleric** weapon it can be a staff, wand, or book.

If you want to make a **Rogue** weapon it can be a bow, dagger, throwing weapon, or sword.

You can also include any desired looks for your weapon.

I actually have a precise formula I use for deciding item level and stats, all you have to do is fill out this :p

I'd like to see the item level kept, any other thing I don't care if you need to change. This will be fun.

Doing it right now, should only take a few minutes :)

Unfortunately Item level is scaled off the players level + rarity, so I can make it player level 27 but I can't guarantee the item level will be 72\. Sorry :(
